| Download the Application
Forms |
 The
Application Packet containing instructions and forms is in Portable
Document Format (PDF). You must have Acrobat Reader 5.1
or higher to download and fill out the forms.

- How to download the Application Packet:
Windows
1. Right-click on the text link below
and release the mouse. A pop-up menu will appear.
2. Choose "Save Target As..." or "Save
Link As.." (depending on your browser).
3. In the "Save As" dialog box, choose
the folder on your hard drive or disk where you'd like to save the
file.
Macintosh
1. Click on the text link below and
hold down the mouse button until a pop-up menu appears.
2. Choose "Download Link to Disk" or
"Save this Link as.."(depending on your browser).
3. In the dialog box, choose "Source"
from the "Format:" pop-up.
4. Choose the folder on your hard drive
or disk where you'd like to save the file.
To Open the Saved Application Packet
To
open the file again, open Adobe Reader and then choose File >
Open. Select the directory and file that you saved the file
to.
- Download the Application
Packet NOTE: Be sure to download to your local
computer first!
