can type directly into the forms online or you may print the forms and
complete by hand or typewriter. Forms cannot be electronically submitted.
Print or make a copy of the completed form(s) and then file the form(s)
with the Clerk’s Office. For more information on filing forms, see Service
of Process.
Requires Adobe Acrobat Reader software You will need Adobe® Acrobat Reader software, version 5.0, or higher, to view, fill out, and print PDF forms. Adobe Acrobat software is compatible with both Macs and PCs. If you already have the Adobe Acrobat Reader, check that it is version 5.0 or higher by selecting “Help,” then “About Acrobat Reader.” If your current version is lower than version 5.0 or if you do not have Adobe Acrobat Reader, you will need to download the software from Adobe’s web site and install it on your computer. This only takes a few minutes, and there are easy-to-follow instructions to help you with the download. NOTE: The blank forms can be saved to your computer but the information you type into the form will not be saved. Be sure to print the form before exiting the page or turning off the computer in order to get a copy of the form with your typed information. To type into the PDF form: The hand cursor
should already be selected when you open the forms. If for any reason
it is not, click the hand in the toolbar at the top of the screen. Move
the mouse over the first blank line in the form. The hand will turn
into an I-beam when it is over the line. Click the mouse and the I-beam
will turn into a blinking vertical line. You can start typing at this point.
How to check a checkbox: Move the hand over the checkbox. The hand will turn into an arrow or a pointing finger. Click the mouse to check or you can use the space bar to check the box. To uncheck the box, move the hand over the checkbox. It will turn into an arrow or a pointing finger. Click the mouse to remove the check. What if I make a mistake? To make a correction, click in the line where you want to make the change. Use the delete key or the backspace key to remove the unwanted text or you can double-click on the line to highlight it. When you type, you will replace the text in that line. When you are finished: Remember, if
you save the form to your computer, it will only save the blank form; it
will not contain the information you filled in. To obtain a copy of the
form showing your typed information, print a copy once the form is completed
by clicking FILE, then PRINT. You may want to print an additional copy
for your records.
