Applicants for a judicial vacancy are required to submit a Confidential Personal Data Questionnaire to the Administrative Office of the Courts . The Questionnaire and instructions are available in Word, WordPerfect or PDF.

Please note that the Questionnaire is updated from time to  time.  It is important, therefore, that the Questionnaire submitted is the current version.  The current Questionnaire was last revised on 02/10/09 as indicated in the upper left-hand corner of the first page of the Questionnaire.
