  RG – most law clerks are on the regular payroll system
  Agency Code:  220100
  Agency Name:  State of Maryland -- Judiciary
  Your SS # and Name
  Check box #1 – Initiate Deposit directly into my checking account
  Enter your bank’s name
  Enter the bank [routing] number and account number from your check. 
         (not off a deposit slip)
  Sign and date the form
