Guidelines for Completing the I-9 Form

Please print pages 7 & 8 - print them front to back on one page.

There are a full set of instructions on the link for you to review; you do not need to bring the instructions page to your Onboarding Session.

  • Section 1 – Employee
    • This part of the form must be completed no later than the date of hire, which is the actual beginning of employment.
    • The form may be completed prior to your start date (at an Onboarding Session with HR or in the judges chambers)
  • Complete
    • Full Name – if you are married enter your maiden name also
    • Current Address
    • Date of Birth
    • Social Security Number
    • E-mail address
    • Telephone number
    • Check one of the boxes
    • Sign your name next to “Employee’s Signature”
    • Date the form next to “Date”
  • Do one of the following:
    1. Take the form with you to your Judge’s chambers if your Onboarding session is scheduled more than 4 days after your start date
      1. Section 2 will be completed by the Judge’s chambers and your ID will be reviewed and copied.
    2. Bring the form with you along with the required original ID and a copy of your ID, to your Onboarding Session if:
      1. Your session is within 3 days of your start date
      2. Your session is before your start date

i.e --  You start on Monday and your Onboarding session isn’t until Thursday – complete Section 1on your first day and take your original ID to the chamber’s where it will be reviewed and a copy made of your ID. Section 2 of the I-9 form is completed by the chambers and then sent to HR with the photo copy of your ID

i.e. – You are starting in August 2012 and you are attending a group Onboarding Session in June 2012– bring the I-9 form with Section 1 completed, your ID and a copy of your ID to the Onboarding Session.  Human Resources will review your ID and complete Section 2 of the form.
