Steps:
Click the Main Menu button.

Step 10Here you are able to read the Maryland Judiciary Grievance Policy.
Step 11Click the Form tab.
Step 12The Employee ID and Location of the employee completing the form will auto-populate.
Step 13Click the Department button.
Step 14Click in the Description field.
Step 15Enter "jis" into the Description field.
In this example it is jis, please enter your correct department.
Step 16Click the Look Up button.
Step 17Click the 0601102 link.
Click the appropriate link for your department.
Step 18Click in the Office Phone field.
Step 19Enter "410-260-7000" into the Office Phone field.
Please enter your office phone number.
Step 20Click the Supervisor ID button.
Step 21Click the 000000003 link.
Please find and choose your supervisor by using the lookup function above.
Step 22Click the Date of Occurance button.
Step 23Click the 3 object.
Choose the date in which the grievance occured.
Step 24Click in the Nature of Grievance/Disciplinary Action Appeal field.
Step 25Enter "I received an unsatisfactory performance evaluation. " into the Nature of Grievance/Disciplinary Action Appeal field.
Enter the nature of your grievance.
Step 26Click the Grievance Type drop-down list.
Step 27Click the Annual Prf Eval-Need Imp/Unsat list item.
Choose the type of grievance you are filing.
Step 28Click in the Remedy Sought field.
Step 29Enter "I would like to be reevaluated." into the Remedy Sought field.
Please type the remedy you would like to see.
Step 30Click in the Prior to filing this grievance, I made the following efforts to resolve this matter field.
Step 31Enter "I have made the required improvements." into the Prior to filing this grievance, I made the following efforts to resolve this matter field.
Type any efforts that you have done to resolve this grievance.
Step 32Click in the I am represented by (name and title of employee's representative if applicable) field.
Step 33Enter "Jane Smith, Judiciary employee" into the I am represented by (name and title of employee's representative if applicable) field.
Please enter your representative's name and title (if you have obtained a representative).
Step 34Click the Save button.
Step 35Click the Preview Approval button.
The next page will show you who the form will go to for approval before it is submitted.
Step 36You may click on the 'Multiple Approvers' links to see the list of approvers in each step.
Click the Submit button.
Step 37You can see that the approval is now 'Pending' for your Administrative Official.
Click the OK button.
