In this topic, you will request authorization for FMLA eligibility.
Steps:
Click the My HR tab.
Step 1Click the Request FMLA Authorization link.
Step 2Using the date fields, enter the FMLA duration you are requesting authorization for.
Step 3The start date defaults to the current date. In this example, enter a future date.
Click the Choose a date button.
Step 4Click the drop-down list.
Step 5Click the May list item.
Step 6Click the 2 link.
Step 7The Expected Retun Date is the end date of the period for which you are requesting FMLA authorization.
Click the Choose a date button.
Step 8Click the drop-down list.
Step 9Click the August list item.
Step 10Click the 19 link.
Step 11Notice additional fields are displayed after you enter the Expected Return Date.
Step 12Click the Absence Reason drop-down list.
Step 13Select the option that most accurately fits your situation. For this example we will use Self.
Click the Self list item.
Step 14Click the FMLA Eligibility link.
Step 15The details of this page are for the FMLA Administrator and/or by the system automatically via validation. You are only responsible for entering the date on which the change will take effect on the section below.
Step 16Click the vertical scrollbar.
Step 17The effective date must be greater than or equal to the Extended Absence Start Date entered on the previous screen. For this example the date is 05/02/2016.
Click the Choose a date button.
Step 18Click the drop-down list.
Step 19Click the May list item.
Step 20Click the 2 link.
Step 21Click the Validate Sections button.
Step 22Click the OK button.
Step 23Click the Return to Extended Absence Request link.
Step 24Enter any comments necessary into the Requestor Comments box.
Step 25Click the Submit button.
Step 26Click the Yes button.
Step 27Click the OK button.
Step 28Click the vertical scrollbar.
Step 29Notice the authorization approval is "Pending".
Note: The extended absence must be approved by Employee Relations before you can enter the FMLA take.
Step 30Notice the status is displayed along with requestor name, the date of the request and any comments included.
Step 31Click the Home link.
Step 32Click the Sign out link.
