04_11_16_Request FMLA Authorization


Concept

In this topic, you will request authorization for FMLA eligibility.


Steps:

  1. Click the My HR tab.

  2. Click the Request FMLA Authorization link.

  3. Using the date fields, enter the FMLA duration you are requesting authorization for.

  4. The start date defaults to the current date. In this example, enter a future date.

     

    Click the Choose a date button.

  7. Click the 2 link.

  8. The Expected Retun Date is the end date of the period for which you are requesting FMLA authorization.

     

    Click the Choose a date button.

  12. Notice additional fields are displayed after you enter the Expected Return Date.

  13. Click the Absence Reason drop-down list.

  14. Select the option that most accurately fits your situation. For this example we will use Self.

     

    Click the Self list item.

  15. Click the FMLA Eligibility link.

  16. The details of this page are for the FMLA Administrator and/or by the system automatically via validation. You are only responsible for entering the date on which the change will take effect on the section below.

  17. Click the vertical scrollbar.

  18. The effective date must be greater than or equal to the Extended Absence Start Date entered on the previous screen. For this example the date is 05/02/2016.

     

    Click the Choose a date button.

  24. Click the Return to Extended Absence Request link.

  25. Enter any comments necessary into the Requestor Comments box.

  30. Notice the authorization approval is "Pending".

     

    Note: The extended absence must be approved by Employee Relations before you can enter the FMLA take.

  31. Notice the status is displayed along with requestor name, the date of the request and any comments included.
