Why do I need to fill out this form? 

Direct deposit means that your paycheck is automatically deposited into your bank account. 
You need to fill out this form so that Central Payroll Bureau (CPB) – they issue our paychecks-- is authorized and has the correct information to ensure your paycheck can be deposited promptly on payday.

Your first check will be a paper check.
