How to Apply

To apply to be an appointed attorney representing indigent defendants during initial appearances, please complete all three of the below forms. All three forms must be signed by an ink signature or a facsimile of the ink signature - typed, digital, or e-signatures are not acceptable.

  1. Complete the Application

  2. Complete the Anti-Discrimination Acknowledgement Form
    • PDF (fillable) 
  3. Complete a Form W-9 (Request for Taxpayer Identification Number and Certification). To properly fill out this form, you must use either an EIN for a business or your Social Security Number if filing as an individual. If using your SSN, place your name on line #1 leaving line #2 blank. If you are using you EIN#, place your business on line #1 leaving line #2 blank. 
  4. Submit all three completed and signed forms in one of the following ways:
